WORK-BASED LEARNING REQUIREMENTS

  1.  To participate in WBL, you must complete the application.  You will receive notification by email if your application is approved.

    1. We will check grades, attendance and discipline.  If you have excessive attendance / tardy issues, disciplinary issues or are failing a required course,  you will not be allowed to participate in the WBL program.

  2. After your application is approved, you must complete the paperwork in the “Readiness” packet to be eligible to leave school and go to work:

    1. Work-Based Learning Agreement

    2. Insurance and Emergency Information

    3. Signed and NOTARIZED Activity Off Campus Contract

    4. Hazardous Occupation Exemption Form ONLY if working in a hazardous environment

 All of these forms must be COMPLETELY filled out, with signatures and dates as shown. PRINT CLEARLY, using blue or black ink.  If you do not have a job yet, leave those lines blank, but all student and parent information should be completed.  I will get signatures from the administration.

You are REQUIRED to earn OSHA Safety Certification before you will be allowed to leave the building.

    1. If you are already certified, you will upload a copy of your certificate or card.

    2. If you are not, you will complete the course before you are allowed to leave to go to work.  The course takes 10-12 hours to complete.  The quicker you finish the certification, the sooner you can leave.

 

Students will CHECK IN or CHECK OUT in Ms. Forsythe’s classroom EVERY DAY (except early dismissal day).  

    1. If you have WBL in the morning, you will CHECK IN on the black tablet when you arrive at school.

    2. If you come to school, then leave for work, you will CHECK OUT on the iPad.  

School attendance is marked at the end of each week based on the report from the tablets, so it is IMPORTANT that you check in or out each day as expected.

 

To maintain your eligibility in the program you must:

  1. complete the class assignments

  2. maintain and upload verification of hours worked and paychecks or paystubs

  3. check in or out every day
  4. maintain good attendance at school and passing grades in any other courses

MINIMUM WORK REQUIREMENTS:

  • 1 block = minimum 10 hours per week

  • 2 blocks = minimum 15 hours per week

  • 3 or more blocks = minimum 20 hours per week

We understand there may be an occasional week with holidays, sickness or other issues that prevent a full week, but the majority of your work schedule should meet these minimums.

 

UNDERSTAND THAT PARTICIPATION IN THE WORK-BASED LEARNING PROGRAM IS A PRIVILEGE, NOT A RIGHT.  You can, and will be, removed from the course and assigned to an on-campus course if you do not meet the requirements of the course.
