Admission to MCHS

Appointments can be made by contacting the school secretary at 731-645-3226.

Please bring the following documents to registration:

*2 proofs of residence (utility bill, driver's license, mortgage statement, rental agreement, etc. )
*Student's social security card
*Student's birth certificate
*Student's immunization record
*Most recent report card from previous school

Also bring these documents if they apply:
*Student's IEP (if they have one)
*Custody papers (if any)
*Transcript from previous school if student will be in the 10th-12th grade

TDOE: Student Records and Confidentiality:
Student directory information for eleventh (11th) and twelfth (12th) graders shall be made available upon request to persons or groups which make students aware of occupational and educational options, including official recruiting representatives of the military forces of the state and the United States. The school shall annually notify parents that the school releases the names, addresses, and phone numbers of secondary students to military recruiters and institutions of higher education unless parents opt out.
